Person directed planning is a service that aligns a CPSS –Adult Planning Consultant with a person with an intellectual disability (18 years and older) to develop a person directed plan.
A person directed plan focuses on the unique gifts, strengths and contributions of the person. One of the objectives of the plan is to develop renewable and sustainable resources, such as support circles, that assist the person to become more included in their community. The Consultant will work with the person and their support circle so that the plan reaches its maximum potential.

Using a variety of center based and community outreach options, we offer therapeutic, recreational and educational opportunities to promote development of physical, emotional, intellectual, spiritual and social skills. Person centered plans are developed for each participant that emphasize increased independence and greater community inclusion.
